Passport Services & Requirements

Hours

  • 10am - 8pm, Monday through Thursday

Birth through 15 years (FORM DS 11)

  • Child must be present
  • 1 passport photo
  • Birth Certificate needed even if a passport is submitted
  • Both parents present with valid, government-issued picture ID, OR one parent present with valid, government-issued picture ID and a notarized letter from the absent parent giving written permission, along with a copy of the consenting parent's ID. ( ID is mandatory )
  • $25 application fee (cash, check or money order payable to: Sterling Municipal Library)
  • $80 passport book ( 4-6 week delivery ) ( Check or money order payable to: US Dept. of State* )
  • $15 passport card ( 4-6 week delivery ) ( Check or money order payable to: US Dept. of State* )
  • $60 expedite fee ( added for 2-4 week delivery ) ( Check or money order payable to: US Dept. of State* )
  • $12.72 (overnight return fee)

Ages 16 and 17 (FORM DS 11)

  • Child must be present
  • 1 passport photo
  • Birth Certificate or passport is submitted
  • One parent present with valid, government-issued picture ID

  • Valid, government-issued picture ID

* Passport fee, expedite fee and overnight fee should be combined into one check or money order per application.
