Sterling Municipal Library is an official Passport Acceptance Agency. Library passport agents accept completed applications during the following hours on a first-come-first served basis:

• 10am - 8pm, Monday through Thursday
• 10am - 5pm, Friday
• 2pm - 5pm, Saturday

Requirements

Birth through 15 years (FORM DS 11)

• Child must be present
• 1 Passport Photo
• Birth Certificate needed even if a passport is submitted
• Both parents present with valid, government-issued picture ID, OR one parent present with valid, government-issued picture ID and a notarized letter from the absent parent giving written permission, along with a copy of the consenting parent's ID. ( ID is mandatory )
• $25 application fee (cash, check or money order payable to: Sterling Municipal Library)
• $80 passport book ( 4-6 week delivery ) ( Check or money order payable to: US Dept. of State* )
• $15 passport card ( 4-6 week delivery ) ( Check or money order payable to: US Dept. of State* )
• $60 expedite fee ( added for 2-4 week delivery ) ( Check or money order payable to: US Dept. of State* )
• $12.85 (overnight return fee)

Ages 16 and 17(FORM DS 11)

• Child must be present
• 1 Passport Photo
• Birth certificate or previous passport required
• One parent present with valid, government-issued picture ID ( ID is mandatory )

Passport fee, expedite fee and overnight fee should be combined into one check or money order per application.
